Purchasing vials for your patients

Although patients have the option to order donors’ vials directly from California Cryobank, you’re also able to place orders on behalf of your patients. 

Placing the order

Healthcare providers can place orders only by calling our team at 866-927-9622, Monday through Friday between 5:00 am and 5:00 pm and Saturday from 8:30 am to 12:30 pm (Pacific Time). For security reasons, you must speak with a representative directly. We cannot accept orders placed using voicemail.

Forms

As a healthcare provider, you must complete the Statement of Physician Responsibility form if you’ll be taking financial responsibility for the purchase of the vials. 

Desired arrival date

Orders can be placed from two business days to 30 days in advance. We recommend that you request your shipment to arrive at least two days prior to use. 

Safe and secure shipping

General shipping information

California Cryobank ships domestically and to more than 35 countries worldwide via our designated carrier to ensure proper handling. All specimens are shipped in liquid nitrogen dry shippers designed to maintain the proper temperature for up to seven days from shipment date. Shipments are pre-paid both ways and can be sent either to your office or your patient's home. Same-day ordering and shipping may be available for an additional fee. Please note that donor vials are shipped from our Arizona facility.

Do I need to complete the Physician Shipment Authorization form for every patient?
The Physician Shipment Authorization form must be completed for each patient if vials should be shipped to one of your clinic's locations.
Once the Physician Shipment Authorization form has been received by California Cryobank, is the vial order or shipment automatically placed?
No. The Physician Shipment Authorization form merely confirms the location(s) you have approved for vial shipment once your patient is ready to schedule it. Vial purchases and shipments can be placed online or by calling our client services team.  
How can I submit the necessary forms?
We accept forms via fax or email. Please be sure to follow the submission instructions listed on each individual form. 

How are the sperm vials packaged and shipped?
We send the vials in liquid nitrogen dry shipper tanks via FedEx or a local courier. Tanks are validated for 7 days from the date shipped. 
If I have limited space, can the vials be shipped to my patient's home instead of my office?
Yes, the vials can be shipped to your patient's home address instead of your office address, excluding international and New York State residents. This can be designated when completing the shipment authorization form. 
How long do the specimens last in the tanks?
The specimens are delivered in dry shipper tanks that are validated for 7 days from the date shipped.
How do we thaw the vial(s)?
Thaw the vials at 37°C / 98°F for 10 minutes in a water bath or 15 minutes in a dry heat block (13mm holes, minimal air around cryovial). Consult the thawing instructions that are included with each tank for further details. 
Who is responsible for returning the tanks?
The person who placed the order is generally responsible for returning the tanks. We ask that all clinics and patients coordinate between themselves to ensure shipment tanks are returned. 
When do the tanks have to be returned?
Tanks should be returned within one week after the receipt of the vials. If the tank is not returned or if it is lost, California Cryobank will charge a lost tank fee.  

How can I download information from a donor’s profile without calling your customer success team? 
You or an individual from your team may register for an account online (on behalf of your clinic) in order to view donors’ information items included in their profiles, including their genetic test summaries (GTS).
