Opening your Account

Follow this link to open your account. It only takes a few minutes and your account will be ready immediately.
You will be prompted to sign the Purchase and Storage Agreement during checkout of your first vial purchase.
However, to purchase vials by phone, please submit the Purchase and Storage Agreement before calling to place your order.
Fax or mail
Start by downloading the enrollment forms:
- Authorization for Release of Semen (A1)
Your physician will complete, sign and fax the A1 back to CCB. This form must be renewed each year. - Purchase and Storage Agreement
This outlines the terms and conditions for utilizing donor semen specimens. It is to be completed and signed by you.
Once you have completed and signed the agreement, return it to us by fax at (310) 443-5259 or by mail:

Your account will be opened within 1 business day of receipt. Please note: Your physician must sign and return the A1 before any vial can be shipped.
Once your account is open, you will be able to register online to check account history, make changes to personal contact information, sign up for email alerts, etc.
To download the documents necessary to establish an account, you will need a copy of Adobe Reader.
