  1. How long does it take to open an account?
    It takes one business day to open an account from the time we receive the paperwork.
  2. What paperwork is involved in opening an account?
    The paper work involved to open an account will depend on the physician's financial responsibility, as explained below. These forms are mandatory to open an account and you must establish an account before you or members of your staff can order a donor specimen. In both cases, the necessary forms can be mailed or faxed to us:
    • If you or members of your staff wish to place orders for your patients and you will be financially responsible for all charges in connection with services provided by California Cryobank, you must only complete the Statement of Physician Responsibility.
    • If you or members of your staff wish to place orders for your patients but you want your patient to be financially responsible, you also have to complete the Authorization for Release of Semen (page A1) and the patient must complete the Purchase and Storage Agreement.
     
  3. Do I need to complete the Authorization for Release of Semen (A1) form for every patient?
    Yes, the Authorization for Release of Semen (A1) form must be completed for every patient to ensure that each patient is receiving appropriate medical care.
  4. Once an Authorization for Release of Semen (A1) has been received by CCB, is an order for specimen automatically placed?
    No. All forms received by CCB are used simply to open an account. You, members of your staff, or your patient must verbally place an order by calling our Client Services Department at 877-422-5279.
  5. Who is responsible for placing specimen orders?
    Specimen orders can be placed either by you, members of your staff, or by your patients. In either situation, you or your patient must first establish an account before placing the orders.
  6. Who is responsible for paying for the samples and shipments?
    If you or members of your staff place the orders, CCB will bill your office. If your patient has an account and places the orders under their account, they are responsbile for payment prior to shipment.
  7. How many vials should I recommend my patients order?
    The number of vials each patient will need varies, and is usually decided by the physician according to the procedure being performed, patient's medical condition and age among other factors.
  8. How do we thaw the specimen(s)?
    The frozen specimen(s) must be placed in 37°C (98°F) water bath for 10 minutes. Any temperature over 40°C (104°F) will severely decrease motility.
  9. If we have damaged a sample and we do not have an account, what can be done?
    You can establish an account and order another specimen(s), or you can order the specimen(s) under your patient's account and reimburse the patient. If you or members of your staff order under your patient's account, your patient will be charged when the order is placed.
  10. How are the specimens packaged and shipped?
    Specimens are packaged and shipped in liquid nitrogen dry shipper tanks via Federal Express or local courier. Shipments within the United States are sent using an overnight courier service.
  11. If I have limited space, can the vials be shipped to my patient's home instead of my office?
    Yes, the vials can be shipped to your patient's home address instead of your office address.
  12. How long do the specimens last in the tanks?
    The specimens are guaranteed in the dry shipper tanks for 7 days from the date shipped.
  13. Who is responsible for returning the tanks?
    The person who placed the order is responsible for returning the tanks. If you or members of your staff have placed an order, then your office is responsible for the tank return.
  14. When do the tanks have to be returned?
    Tanks should be returned within one week after the receipt of specimens. If the tank is not returned or if it is lost, CCB will charge for the tank.
  15. What are your quality assurance methods?
    Every aspect of our operation at California Cryobank involves extensive quality assurance systems. Our quality assurance systems include:
    • Biometric identification device that identifies donors by their handprint.
    • Two-part color coding system that verifies that clients receive the correct specimen.
    • Density gradient processing to prepare IUI ready donor specimens, which removes cells that are associated with bacteria, viruses and infection; pre-selects sperm that have superior motility; pre-selects sperm with a lower incidence of DNA anomalies; and removes seminal plasma, thus reducing post insemination cramping.

  17. Do you offer financial assistance to those patients in need of donor semen?
    Yes, CCB offers payment plans for donor semen services and semen storage clients. Please have your patient contact our Client Services Department for more details at 866-927-9622.

  20. Do you have an Open Donor Program?
    Yes, California Cryobank does offer an Open Donor Program. CCB Open Donors agree to have at least one contact with the child when the child turns 18.
